Please follow the instructions below to begin the digital benefit selection process on Clark Land’s Employee Navigator website. The effective dates of your selection will be provided after you complete the process.

Please note that unless there is a qualifying event during the 2019-2020 plan year (loss of other group coverage, the addition of a dependent, etc.), this will be your only opportunity to make changes until the next plan year beginning October 1, 2020.

 

Let’s Get Started…

First, create your Employee Navigator website account, login at https://www.employeenavigator.com/benefits/Login/Registration.aspx and enter the required information.

Please note – you will be asked for the Company Identifier (it is CLResources).

Next, follow the instructions to elect your medical, dental, and vision benefits for the 2019-2020 plan year. (Please remember to finish any required tasks listed as well.)

  • This year we are excited to offer you many robust plan options. You have eight different plans to choose from for your medical needs; including a new United Health Care Silver Signature Value HMO. Also, there is a new and improved Vision benefits package with VSP.
  • Last but not least, Clark Land Resources is happy to announce the implementation of a new pet benefit. Pet Benefit Solutions offers a 3-tier approach to providing your pet with the best possible care. This benefit is available to all employees. Enrollment is voluntary and paid 100% by the employee. You may choose to enroll in any or all of the following: Pets Best Pet Health Insurance, Pet Assure Veterinary Discount Plan, or PetPlus Prescription Discount Plan.
  • There were a few minor changes to the employee contribution amount this year. Employees will be responsible for $25 per paycheck for all health insurance plans except for the Anthem PPO and the new UHC Signature Plan, for those plans this amount will be $50 per paycheck.
  • All other plans for dental, life, AD&D, and long term disability will remain the same.
  • Again, on the Employee Navigator website, please remember to finish any required tasks listed during the process.
  • Your annual HIPAA/DOL Documents will be posted on your personal homepage for your review. You may print the documents if you wish.

Anthem Blue Cross Silver Select PPO B 2019-2020

This plan requires members to select a primary care physician (PCP) and obtain a referral to see specialists.  Typically, you can only visit doctors and facilities that are part of the network.

  • You can go to a participating physician without a referral
  • The payroll cost for this plan is the highest
  • Your cost at the time of care is low
  • You can visit providers who do not participate with the plan

Kaiser Bronze HMO C (HSA Eligible) 2019-2020

This plan requires members to select a primary care physician (PCP) and obtain a referral to see specialists.  Typically, you can only visit doctors and facilities that are part of the network.

  • Plan requires members to pick a doctor for routine care and get a referral to see a specialist
  • The payroll cost for this plan is low
  • Your cost at the time of care is higher
  • You cannot visit providers who are not on the approved list

Kaiser Silver HMO C 2019-2020

This plan requires members to select a primary care physician (PCP) and obtain a referral to see specialists.  Typically, you can only visit doctors and facilities that are part of the network.
